Copper-based Intrauterine Contraceptive System

A copper-based intrauterine contraceptive device (IUCD) is a small device made with copper that is inserted into the uterine cavity to prevent pregnancy for upto 12years.

How it works

Copper IUCDs work to prevent pregnancy by releasing copper in the uterus and preventing fertilization.

Effectiveness

  • When inserted correctly, Copper-based IUCDs provide 99% effectiveness.

Advantages

  • Copper IUCDs have high effectiveness and safety and provide immediate protection after insertion.

Progestin-only Contraceptive Pills are also known as “the mini-pill” and only contain one hormone – progestin, in a smaller dose (typically 10% – 50%) less than that used in the combined pill.

How it works

The pill works by preventing the release of eggs from ovaries and thickening the cervical mucus, therefore preventing the sperm from meeting an egg.

Common Types of POPs

The common types of POPs contain 35 active pills in a single pack cycle.

Effectiveness

  • When used consistently and correctly, POPs are 99% effective in preventing pregnancy
  • Effectiveness depends on the user: For women who have monthly bleeding, risk of pregnancy is greatest if pills are taken late or missed completely.

Hormonal Method
Progestin-only Contraceptive Implants are commonly referred to as “Implants”. Implants are one or two small flexible rods that contain the progestin hormone and are inserted under the skin of a woman’s upper arm to give protection against pregnancy for between 3 to 5 years.

How it works

The hormones work by preventing the release of eggs from ovaries and thickening the cervical mucus, which interferes with sperm transport.

Common Types of Implants

The common Implants available provide pregnancy prevention for three years, four years and five years.

Effectiveness

  • Implants provide 99% effective protection against pregnancy.

Advantages

    • Implants are highly effective and offer long-term protection against pregnancy for upto 3 to 5 years.
    • Suitable for women who wish to avoid daily, weekly and monthly contraception regimen.
    • In addition, fertility returns almost immediately after implants are removed.

Limitations

  • The client cannot initiate or discontinue the method on her own as it requires a trained provider to insert and remove the implant.
  • When using this method, you may experience some changes in bleeding patterns for example irregular bleeding, heavy or prolonged bleeding, or spotting.
  • Some women also experience weight changes, headache, dizziness, mood swings, abdominal bloating, acne, or breast tenderness.

The Lactational Amenorrhea Method (LAM) is a temporary method of preventing pregnancy for women who have recently given birth

How the method works

  • LAM is based on the natural effect of breastfeeding on temporarily stopping ovulation (fertility).
  • Prolactin released during continuous breastfeeding suppresses ovulation which makes pregnancy unlikely.

Effectiveness

  • LAM is up to 99% effective if practiced during the exclusive breastfeeding period.
  • It’s important to note that LAM’s effectiveness depends on 3 conditions; the mother’s monthly bleeding has not come, the baby is fully and oftenly (day and night) breastfeeding, and the baby is less than 6 months old.

Advantages

  • LAM provides effective protection against pregnancy as long as all the three LAM criteria are met.
  • Return to fertility is immediate once you stop exclusive breastfeeding.
  • LAM does not interfere with sexual activity and it has no known health risks or direct costs.

Limitations

  • For LAM to work, all three of the following must be true: the woman’s menstrual periods have not resumed, the baby is exclusively breastfed, and the baby is less than six months old.
  • It is important to note the effectiveness of LAM is reduced in the absence of exclusive breastfeeding.
  • Female condoms can also be made less effective by heat, light, and humidity.
  • In addition, one may experience skin irritation or condom breakage.

Combined Oral Contraceptive Pills (COCs)

No. There is no evidence that taking a “rest” is helpful. In fact, taking a “rest” from COCs can lead to unintended pregnancy. COCs can safely be used for many years without having to stop taking them periodically.
Not always.Researchers have not found a direct link between using hormonal birth control and gaining weight. Every hormone method can affect you differently.1

Progestin-only Contraceptive Pills (POPs)

Yes, she does not need to wait until 6 weeks postpartum. POPs are safe for both the mother and the baby and do not affect milk production.1
Yes.There is no minimum or maximum age for POP use.1

Progestin-only injectable contraceptives

No. There may be a delay in regaining fertility after stopping progestin- only injectables, but in time the woman will be able to become pregnant as before, although fertility decreases as women get older.

Combined Patch

If the patch was off for less than 2 days, apply a new patch as soon as possible. However, if the patch was off for more than 2 days, apply a new patch as soon as possible and also use a backup method such as condoms for the next 7 days. If during week 3, skip the patch-free week and start a new patch immediately after week 3. If during week 1 and unprotected sexual intercourse occurred in the past 5 days, consider taking emergency contraceptive pills.

Fertility Awareness Methods

“Fertility awareness” methods are a method of contraception where a woman tracks and knows how to tell when the fertile time of her monthly cycle starts and ends. This is also sometimes called periodic abstinence or natural family planning. A woman can use several ways, alone or in combination, to tell when her fertile time begins and ends

Calendar-based methods

  • Calendar-based methods are where a woman keeps track of days of the menstrual cycle to identify the start and end of the fertile time.
  • One example is the Standard Days Method, where a woman avoids unprotected sexual intercourse on days 8 through 19 of the menstrual cycle.
  • Another example is the calendar rhythm method.
 

Symptoms-based methods

  • Symptoms-based methods depend on tracking an observing signs of fertility in a woman’s body, for example:-Cervical secretions: When a woman sees or feels cervical secretions, she may be fertile. She may feel just a little vaginal wetness. -Basal body temperature (BBT): A woman’s resting body temperature goes up slightly after the release of an egg (ovulation). She is not likely to become pregnant from 3 days after this temperature rise through the start of her next monthly period.
  • Examples of symptoms-based methods are the Two Day Method, BBT method, ovulation method (also known as Billings method or cervical mucus method), and symptothermal method.
 

How the method works

  • Fertility awareness methods work by helping a woman know when she could become pregnant.
  • During the fertile days, a woman prevents pregnancy by avoiding unprotected sexual intercourse, by abstaining altogether or using other contraceptive methods such as condoms or a diaphragm. |

    With correct and consistent use, the effectiveness of various fertility awareness methods in preventing pregnancy are as below:
    • Standard days method – 95%
    • TwoDay method – 96%
    • Ovulation method – 97%
    • Symptothermal method – 99%
    With common usage, the effectiveness of various fertility awareness methods in preventing pregnancy are as below:

  • There are no side effects or known health risks to using fertility awareness methods.
  • This method does not require medical procedures and usually does not require purchasing supplies.

  • It also allows some couples to adhere to their religious or cultural beliefs about use of contraception.
  • Fertility awareness methods can be used both by women who want to prevent pregnancy and women who want to become pregnant as it helps them track the most fertile days of their cycles.

Limitations

  • It requires very careful and consistent tracking and adherence to be most effective at preventing pregnancy.
  • If a woman’s menstrual cycles are irregular, identifying the fertile time may be difficult. For example, menstrual cycle irregularities are common in young women in the first several years after their first monthly bleeding, and in older women who are approaching menopause.

Hormonal Method
Emergency contraception (ECs) are contraception methods which can be used if unprotected sex has taken place or when another method of contraception has failed (e.g. a torn condom or forgotten pill).

How it works

  • ECs work by preventing or delaying ovulation and inhibiting or slowing down the transportation of the egg and sperm through the fallopian tubes, which prevents fertilization and implantation.

With so many birth control options available, choosing a most suitable form can be discouraging for many teens. We support youth worldwide to answer initial questions: Which method protects against STIs? What about convenience of use? How effective are they?

In 2021, The Ministry of Health (MoH) in collaboration with stakeholders hosted an event whose theme was ‘Sustaining Family Planning Services during the COVID-19 pandemic while marching towards sustainable domestic financing of Family Planning in Kenya’.

The occasion was used to raise awareness on how Family Planning choices and opportunities have changed for girls and women since the COVID-19 pandemic. It was a platform to mobilize support and goodwill for prioritization and continuation of Family Planning services during the COVID-19 pandemic.

Kenya has made outstanding progress in increasing access to modern methods for fertility management from 36% of married women using modern methods of family planning at the launch of the World Contraceptive Day campaign in 2007 to a current regional high of 62%. The Kenyan Government is committed to providing quality family planning services. In 2019 and 2020, UNFPA mobilized USD. 4.8 Million and US$ 3.1 million respectively towards Family Planning  intervention in Kenya through the Ministry of Health.

With support from various international partners and the Government of Kenya, previous efforts have translated to the prevention of over 1,464,068 unintended pregnancies, nearly 1,439 maternal deaths and aversion of 890,168 unsafe abortions over the next three years.

Providing access to contraceptives and reproductive health services helps women thrive and prosper in economies hit by the COVID 19 pandemic.There is a need for strong investment in Family Planning as an intervention for saving lives.

Male Condoms

The male condom is a thin, latex rubber sheath made to fit a man’s erect penis.

How the method works

  • Condoms prevent pregnancy by physically preventing the male sperm from gaining access to the woman’s reproductive system.

Common Type of Male Condoms

  • Condoms come in different sizes, colors, and textures. Some are coated with a lubricant or spermicide.

Effectiveness

  • When used consistently and correctly in every sexual encounter, male condoms can be 98% effective.
  • However, typical male condom use is only moderately effective at about 87%.

Advantages

  • Generally easy to obtain.
  • Can easily be carried and used on demand
  • It can be used when breastfeeding.
  • It is important to note that the male condom prevents pregnancy, HIV/AIDS and STIs spread by discharge and skin-skin contact.

Limitations

  • Cannot be used with oil-based lubricants since this could reduce their effectiveness.
  • If not properly used, condoms may tear or come out duirng sex.
  • Condoms can also be made less effective by heat, light, and humidity.

Combined Oral Contraceptive Pills (COCs)

Hormonal Method

  • Combined oral contraceptive pills (COCs) are often shortened to “the pill” or “pills”.
  • These pills contain synthetic hormones estrogen and progestin which are similar to natural homes produced in a woman’s body.

How it works

The pill works primarily by preventing the release of eggs from ovaries (ovulation).

Common Types of COCs

The pill is usually provided in packs of 21 or 28 tablets. In the 28-pill pack, only the first 21 pills are active, with the remaining seven being inactive.

Effectiveness

  • When used consistently and correctly, the pill is 99% effective in preventing pregnancy.
  • Effectiveness depends on the user: Risk of pregnancy greatest when a woman starts a new pill pack 3 or more days late, or misses 3 or more pills near the beginning or the end of a pill pack.
  • For greatest effectiveness, a woman must take one pill every day.

Advantages

    • COCs are highly effective if used correctly and consistently.
    • No delay in return to fertility after the pill is stopped.
    • Help to reduce heavy and painful periods.

Limitations

  • It is important to note that the pill must be taken daily at the same time each day, whether or not a woman has sex that day.
  • Does not protect against HIV infection (AIDS) and other sexually transmitted infections (STIs).
  • May cause changes in bleeding patterns.
  • When using the Pill, some women may experience nausea, spotting, or bleeding in between menstrual periods, mild headaches, breast tenderness, weight change, mood changes, and lack of a monthly period.

Fertility Awareness Methods

“Fertility awareness” methods are a method of contraception where a woman tracks and knows how to tell when the fertile time of her monthly cycle starts and ends. This is also sometimes called periodic abstinence or natural family planning. A woman can use several ways, alone or in combination, to tell when her fertile time begins and ends

Calendar-based methods

  • Calendar-based methods are where a woman keeps track of days of the menstrual cycle to identify the start and end of the fertile time.
  • One example is the Standard Days Method, where a woman avoids unprotected sexual intercourse on days 8 through 19 of the menstrual cycle.
  • Another example is the calendar rhythm method.

Symptoms-based methods

  • Symptoms-based methods depend on tracking an observing signs of fertility in a woman’s body, for example:

    -Cervical secretions: When a woman sees or feels cervical secretions, she may be fertile. She may feel just a little vaginal wetness.

    -Basal body temperature (BBT): A woman’s resting body temperature goes up slightly after the release of an egg (ovulation). She is not likely to become pregnant from 3 days after this temperature rise through the start of her next monthly period.

  • Examples of symptoms-based methods are the Two Day Method, BBT method, ovulation method (also known as Billings method or cervical mucus method), and symptothermal method.

How the method works

  • Fertility awareness methods work by helping a woman know when she could become pregnant.
  • During the fertile days, a woman prevents pregnancy by avoiding unprotected sexual intercourse, by abstaining altogether or using other contraceptive methods such as condoms or a diaphragm. |

  • With correct and consistent use, the effectiveness of various fertility awareness methods in preventing pregnancy are as below:

Progestin-only injectable contraceptives

Hormonal Method

Progestin-only injectables (also called POIs) are contraceptives that contain the progestin hormone and provide protection from pregnancy following an injection by a medical professional. They are given by injection of the progestin into the muscle or under the skin.

How it works

POIs work by preventing the release of eggs from a woman’s ovaries.

Common Types of POIs

  • POIs are broadly categorised based on their intervals of injection.
  • The most widely used injectables include; the monthly POI interval injection, the two months POI interval injection and the three months POI interval injection.

Effectiveness

  • If injections are done consistently and correctly, POIs are 99% effective in preventing pregnancy.
  • However, they are 96% effective as commonly used; that is, when a woman is late for an injection or misses an injection.

Advantages

    • When correctly and consistently used, POIs are highly effective and safe.
    • POIs do not require daily or weekly attention.
    • POIs are private: no one else can tell that a woman is using contraception.
    • POIs do not affect breast milk production hence can be used during breastfeeding.

Limitations

  • This method requires a woman to go to a health care facility regularly as it is given by medical professionals.
  • Return of fertility may be delayed for four months or longer after discontinuation.
  • Does not protect against HIV infection (AIDS) and other sexually transmitted infections (STIs).

Side Effects experienced by some users

  • Changes in bleeding patterns for example irregular bleeding, heavy or prolonged bleeding, or spotting
  • Weight changes, headache, dizziness, mood swings, abdominal bloating, acne or breast tenderness
